故事發生在第一集之後的6個月之後，瑞切爾·凱勒（納奧米·沃茨飾）在西雅圖經過連番恐怖事件之後，希望逃離這段恐怖的記憶，為此她帶着兒子艾丹（大衞·多夫曼飾）遠遷到寧靜美麗的沿海城市，俄勒岡的艾斯特里亞，希望這裏的風光能慢慢醫治留在兩個人心裏的陰影。
然而恐怖總是如影隨形，瑞切爾希望逃避恐怖的信心很快被當地發生的一起罪惡粉碎，在罪案現場，警察發現了一卷神秘的沒有題目的錄影帶，一切看起來都是那麼熟悉：詛咒再次復生，恐怖加速蔓延……
瑞切爾在這裏剛剛找到一份新工作：新聞記者馬克思·洛克（西蒙·貝克飾）的助手，在一樁新聞事件的調查中，瑞切爾在警察局檔案室中發現了一個非常神秘的“X檔案”，這是一個年輕女孩的離奇死亡案，案件至今未破，現場的證據只有一盤神秘的錄影帶。
就在瑞切爾的調查進一步展開的時候，艾丹突然出現生病，全身發冷並且皮膚上出現許多不明原因的傷痕，醫生將這些作為孩子遭受虐待的證據，而瑞切爾知道，這是兇鈴作祟，貞子再次返回，殘忍地繼續這個死亡和恐怖的循環遊戲。現在，要挽救自己的孩子，瑞切爾只有再次回到西雅圖，繼續調查貞子的身世之迷……

·片中瑟姆拉的黑色長髮是用真人頭髮製成的假髮。
·為了扮演莎瑪拉，凱麗·斯泰伯斯不得不戴上白色隱形眼鏡。
·因為野性難訓，影片拍攝中沒有使用真實的鹿，片中的鹿都是用電腦生成的。另外，在古代日本，鹿是神的使者，要像珍寶一樣保護。
·納奧米·沃茨親自完成了大部分井中的攀爬場景。
·為了讓瑟姆拉在井中的動作越發詭異，劇組請來了精通柔術的特技演員邦妮·摩根來擔任替身。
·製片方最初曾希望由商業電視導演諾姆·穆羅來執導本片，但後者因意見分歧而放棄。
·理查德·凱利也曾為該片導演的人選。
·在拍攝狂歡節的場景時，當地人誤以為是真正的狂歡節而一起加入，他們變成了影片中的臨時演員。
